Hospice Care at Rivercross
What is Hospice Care?
Hospice care is for individuals who are nearing the end-of-life. The services are provided by a team of health care professionals who maximize comfort for a person who is terminally ill by reducing pain and addressing physical, psychological, social, and spiritual needs. To help families, hospice care also provides counseling, respite care and practical support. The goal of hospice is to support the highest quality of life possible for whatever time remains. Hospice care decreases the burden on family, decreases the family’s likelihood of having a complicated grief experience and prepares family members for their loved one’s death. Our hospice team provides care in the patient’s home, wherever that may be including a retirement community, assisted living or long-term care facility.
Rivercross Healthcare is locally owned and operated. Because our roots are in this area, we understand the needs of our family, friends, and neighbors.
Rivercross Healthcare evaluates and responds by offering the level of care that best meets the needs of each individual patient. We work together with the patient, family, patient’s attending physician, and hospice medical director to decide the most appropriate level of care. The levels of care provided by Rivercross Healthcare are as follows:
Routine Home Care
- Over 96% of all hospice care is provided intermittently in the comfort and security of the patient’s home.
General Inpatient Care
- Utilized only in emergencies for pain control and symptom management within a specialized inpatient unit or a contracted Medicare-certified skilled nursing facility or hospital.
Respite Care
- This level of short-term care is used when necessary to relieve the family members or other persons caring for the individual.
Continuous Care
- Used for a period of crisis, this level of care is covered as necessary to maintain the patient in their current living environment.
